Now we’re making it even more fun for you to share a smile with our patient referral drawings. All you have to do to participate is refer your friend (be sure to get their permission first) below. If your friend attends a consult with us, you (and your friend) will be entered to win our current prize.

• Will run through June 30th, 2021 and the prize will be awarded on 6/30/21.
• To enter you must refer a friend to our office. They must schedule and attend a free consultation in order for you to be entered.
• The person who was referred will also be entered to win
• The referral must be new to our office (cannot be in our observation program already, but can be a sibling or parent)
• You can be entered multiple times - every time you refer someone and they schedule a consultation
• The prize will be non-transferrable
